Efek Pemberian Probiotik, Nigella Sativa, lmunoterapi terhadap CD4+IL-4+, CD8+11.-4+ dan Skoring Asma (Effect of Probiotic, Nigella Sativa and immunotherapy to CD4+1L-4+, C08+ IL-4+ Expression, and Asthma Scoring

ABSTRACT

CD4.1L-4+ cells play a role in asthmatic inflammation. The role of pro inflammatory CD8+in asthma is associated with Tc2 subset that produces IL-4+ Immunotherapy is a useful asthma treatment and immunomodulatory is considered to improve its effectiveness. So far, there has been no study evaluating the long-term administration of immunotherapy, probiotics, and Nigella sativo on the modulation of immune responses, particularly C04+11.4:. C08.1L4; and asthma scoring.

The research design was experimental randomized clinical trials (RCTs), post-test control study for CD4+IL-.4+CD8+IL4+, and pre-post control study for scoring asthma. Subjects were divided into 4 groups, i.e. immunotherapy+placebo, immunotherapy+Nigeila sativa, immunotherapy+probiotics, immunotherapy+Nigella sativa+probiotics, with treatment over 56 weeks. immunotherapy used was house dust mite (HDM) subcutaneous immunotherapy. Probiotics given were ProBi (Medifarma) containing 2x109 colony forming units (cfu)/g Lactobacillus acidophilus and Bifidobacterium lactes. Asthma scoring was assessed with Asthma Control Test (ACT) score, in which skor >19 indicates controlled asthma. The numberofCD4+1L4+and CD8+IL4+was measured using flowcytometry.

The results showthe numberof CD4+IL.4+ and CD8+ IL4+ do not differ significantly between groups. The lowest C04+IL4+and the highest War expression are found in immunotherapy+Nigella sativa+probiotic group. Asthma scoring increases significantly after treatment in immunotherapy+Nigella sativa (p=0,002), immunotherapy+probiotic (p=0,000), and immunotherapy+Nigella sativa+probiotic (p=0,000). Immunotherapy+probiotic group has higher asthma scoring than immunotherapy (p=0,04). It can be concluded that administration of immunotherapy + Nigella walla + probiotics decrease the number of CD+IL4+ lowerand increase the number ofCD8+IL4+although insignificantly higherthan immunotherapy alone. Giving combination of immunotherapy+probiotics improve asthma scoring betterthan immunotheropyai alone.
