The Management of Rectovaginal Fistule in RSUPD Dr. Cipto Mangunkusumo during 1985-1996 (Penatalaksanaan Fistula Rektovaginal di RSUPD Dr. Cipto Mangunkusumo Tahun 1985-1996)
Objective: To evaluate the management of the cases of rectovaginal fistuale in Dr Cipto Mangunkusumo Hospital from 1985 to 1996. Design/data identification: Restropective description. Setting: Division of Reconstruction Urogynecology, Department of Obstetrics and Gynecology, Faculty of Medicine/Dr. Cipto Mangunkusumo Hospital. Subjects, patients, participants: Patients with rectovaginal fistulae. Intervention: Main outcome measures: The success of rectovaginal fistulae operation. Results: In 1985-1996, there were 10 cases of rectovaginal fistulae managed at the Department of Obstetrics and Gynecology, Faculty of Medicine, University of Indonesia/Dr. Cipto Mangunkusumo Hospital. Most of the patients were 21-30 years of age (70 percent), 90 percent cases were assisted by medical personnel and paramedics, and 1 case assisted by traditional birth attendant. In the only case helped by the traditional birth attendant. In the only case helped by the traditional birth attendant, there was found rectovaginal fistulae, 6 cases were referrals and the other 4 cases occured in Dr. Cipto Mangunkusumo Hospital. There were two cases operations have been at tempted outside of Dr. Cipto Mangunkusumo Hospital, but these operation failed (recurrence). Reparation was conducted for ten cases, and with one-time reparation it proved to be successful. Conclusions: (1) The cases of rectovaginal fistulae were relatively few, approximately 1 case each year in Dr. Cipto Mangunkusumo Hospital. (2) Ten cases of rectovaginal fistulae managed in Dr. Cipto Mangunkusumo Hospital, all turned out to be successful. (3) Thus, rectovaginal fistulae primarily as a result of episiotomy is not sufficient. (4) The assessment of location, size of fistulae, timing of preperation, and the acquistition of adequate operation technique are the determinants of success in fistulae operations.
