Clinical pharmacology is an academic discipline in medical field. The substance is build from the combination of basic pharmacology and clinical practices in medicine. It has been grown with the increase in both the number and complexity of drugs. The primary aim is to promate the safety and efficacy of rational drug use in order to optimise benefits and minimise risks. The background of its existence emerges from : - The central role of drug therapy in medical field. - The irrational use of drugs has become more and more create problems in advers drug reactions as well as the cost in health services. - The need to improve rational drug policy and drug use. In the erar of clinical pharmacology some importance steps has been formulated and is beeing conducted as : - Essential drug concept, - 3 basic strategies : regulation, education and management, - 5 criterions in operational drug use : accuracy in indication, patient, drugs, doses and becareful in adverse drug reactions, abbreviated : 4T + 1W (Indonesia).
