Theophylline Serum Level Control and Lung Function Test on Asthma Patient Managed due Standard Protocol in Lung Department Dr. Soetomo Hospital, Surabaya (Pemantauan Kadar Teofilin Serum dan Pemeriksaan Faal Paru pada Penderita Status Asmatikus yang Dikelola dengan Protokol Standar di UPF Paru RSUD Dr. Soetomo, Surabaya)
A study has been done on the theophylline serum levels and pulmonary function tests, comparising FVC, FEV1 and PEFR, on 21 patients with status asthmaticus, age 15 to 50 years, admitted in the Intensive Care Unit, Pulmonary Department Dr. Soetomo General Hospital Surabaya and treated with the standard protocol. Theophyllines serum levels and pulmonary functions test were measured immedistely after admission, at 7 am, 1 pm and at 7 am on the following days and also at 2 and and 4 hours after all treatment was stopped. Theophylline serum levels before treatment was 8.212 plus/minus 5.125 ug/ml. Whereas after treatment the theophylline serum levels increased to 20.369 plus/minus 4.861 ug/ml, and the theophylline serum levels 4 hours after treatment stopped was 12.739 plus/minus 3.739 ug/ml. With the standard protocol, the therapeutic theophylline serum level was already obtained and in 50 percent of the patients the therapeutic theophyline serum level was exceeded. Minor theophylline side effects such as nausea and vertigo, occured in 3 of the 21 patients. Pulmonary function tests improved in all patients. A 15 percent improvement of the FEV1 occured within 7 hours and 54 minutes, and a 20 percent improvement of the PEFR occured within 8 hours and 58 minutes. Average length of stay was 38 hours and 17 minutes.
