Malignant neuroleptic syndrome (MNS) is supposed to be an antipsychotic drugs toxic reaction which sign are hypethermia, rigidyty, awakening decrease, hypothalamus disturbance, and neuromuscular disturbance. The diagnosis of MNS is made base on diagnostic criteria of Pape, Keck, McEllory (1989). The clinical symptom of MNS is similar to sepsis or meningoencephalitis. During period at April 1992 to February 1993, five cases of MNS were occupid at Department of Neurology in Kariadi Hospital, Semarang. The patients had anti phychotic treatment hystory (haloperidol, trifluoperazine, or chlorpromazine), before MNS signs appeared. The patients with autonom disturbance (blood pressure lability) is showed poor prognosis. The mortality is very high (80 percent) because of acute renal respiration.
