Epidemiological Study in Dengue Hemorrhagic Fever on Adolescence (Studi Epidemiologis Demam Berdarah Dengue pada Orang Dewasa)
Case proportion of DHF in the adult tend to increase, and for this purpose, a study to know about clinical aspect of DHF in the adult was carried out to optimize the management of the patient. This a prospective study done during 12 months (Feb, 1990-Jan, 1991) among patients with clinically and serologically DHF positive hospitalized in the Department of Internal Medicine Dr. Kariadi Hospital Semarang. The data were analized descriptively. During that time, there were 138 patients diagnosed as clinical DHF in which 85 were confirmed by positive serological test (61.59 percent). DHF is the third rank among infectious disease hospitalized i.e. 9.39 percent after Typoid Fever (34.76 percent) and Gastroenteritis (32.61 percent). DHF most frequently occure at the age of 15-19 years as much as 46 patients (54.12 percent) with a proportion between male and female 1:1.43. The most common symptoms were fever (100 percent), then vomiting 81 patients (95.29 percent), and epigastric pain 75 patients (85.71 percent). The clinical signs including fever 83 patients (97.65 percent), positive RL 76 patients (89.41 percent) and epigastric pain 68 patients (80 percent). Laboratory test revealed hemoconcentration 67 patients (78.82 percent), a high HI test 60 patients (70.58 percent), and thrombocytopenia 52 patients (61.18 percent). Interpretation on HI test found presumptive infection is 55 patients (64.71 percent) with none of primary infection detected. Complications including shock 20 patients (23.53 percent), ARF 12 patients (14.12 percent), and DIC 7 patients (8.23 percent). Lenght of stay ranged from 4-6 days needed in 40 patients (47.06 percent). The mortality rate was 5.88 percent all of them were female who suffered DHF grade IV with cause of death was DIC. There was no significant different of clinical pattern between adult and infant patients of Dengue Hemorrhagic Fever.
