Pengetahuan dan Praktek Bidan Desa Pasca Pelatihan Deteksi Dini Kasus GAKI di Kabupaten Temanggung (Knowledge and Practices of Village Midwives in Iodine Deficiency Disorders (IDD) Cases after Early Detection Training in Temanggung Regency)
Iodine deficiency caused cretin, hypothyroidism, and unreparable cognitive developmental damage. Iodine deficiency could be detected early through hypothyroidism screening in newborn. However, in Indonesia this program is not a compulsory procedures. The system of maternal and child health services in health center (posyandu) made it possible to conduct the screening of infants. The village midwives have responsibility to care for pregnant and post partum women and newborn for early detection until one year old. Early detection of iodine deficiency disorders (IDD) is very important to eliminate new cretinism. To recognize the symptoms of hypothyroidism in infants a training was provided.
This study aimed to determine the ability of midwives to identify clinical symptoms according to neonatal hypothyroid index (NHI) and taking bloodspot samples in newborn and to identify the obstacles in training.
Observational study with qualitative design. Participants were eight village midwives from four districts at Temanggung regency with focus group discussions (FGD) as method of data collection. It was conducted after three months training on screening and early detection of IDD cases. Data was analyzed qualitatively used Miles and Huberman method with support of quantitative data that were results of clinical observations and bloodspot TSH testing to determine suitability results of clinical observation with bloodspot TSH testing.
Village midwives had no sufficient knowledge and skills in early detection of IDD cases through the introduction of clinical symptoms and taking bloodspot sample in the community. In clinical detection practice of IDD, they still had trouble in determining a fontanella minor in newborns. In bloodspot sample taking, they are less skilled because the lack of practice and experience.
Village midwives had not sufficient capability in early detection of IDD cases in society that further training in skills and knowledge of early detection of IDD are needed.
