Permasalahan Pemberian Obat pada Pasien Geriatri di Ruang Perawatan RSUD Saiful Anwar Malang (Drug-Related Problem in Hospitalized Geriatric Patients at Sal-fill Anwar Hospital Malang
ABSTRACT
Drug-related problems often occur in hospitalized patients and the most common risk factors are the presence of polypharmacy and increasing of age. This research aims to determine the description of age, sex, number of drugs and the prevalence of Drug-Related Problems (DRP) in hospitalized geriatric patients.
This research is a retrospective cohort study with a secondary data collection in geriatric patients (> 60 years) at Saiful Anwar Malang Hospital during the period of January to December 2011. It was found that from 150 subjects, 55,3 % were male, age range of 60-65 years old was 28%, 66-70 years old was 36%, 71-75 years old was 19,3%, >7.5 years old was 16.7%. Polypharmacy occurred in 72% of geriatric patients, DRP prevalence by 72% of population. Common drugs used were antibiotics (73,3%), 112 antagonists (60,7%), antihypertensives (46%), antiemetics (37,3%), NSAiDs (32%). Most common type of DRP is the potential for drug interactions (66%), inappropriate dose (17.394 unnecessary drugs (16%), adverse drug reaction (14%), and inappropriate drugs (8,7%). The most potential drug interactions were in ACE-inhibitor, diuretics and antiplatelet. Drug duplicationoccurs on administration ofPA:sand H2 antagonists.
There was a significant correlation between the increasing numberofdrugs given to the prevalence of DRP (p=0,000). However, there was no significant correlation between increasing age ofgeriatric patients to the prevalence ofDPP (p=0,366). Relative risk of geriatric patients with polypharmocy experienced byDRP is 1,822 (OM: 1.291-2,569).
