Diagnosis dan Tata Laksana Vertigo pada Sindrom Stroke (Diagnosis and Management of Vertigo in Stroke Syndrome)
ABSTRACT
Vertigo (or dizziness) and equlilibrium disorders are the third most common symptoms in emergency ward.They can be caused by peripheral or centra pathologic origin.There is an overlap between symptoms of acute vestibularsyndrome with acutevertebro-basilar syrdrorne.Symptoms associated with posterior circulation disorders have wide variations depends on occluded arteries (PICA [posterior inferior cerebellar artery] AICA[anterior carebellar artery]or SCA [superior cerebellar artery) as seen in Wallenberg syndrome or lateral irferior pontine syrdrome. Diferences between peripheral vertigo symptoms such as in Meniere's syrdrome. BPPV, vestibular neuronitis and lbyrinthitis and central vertigo-related symptoms in the vertebro-basilar insufficiency need to be understood. Laboratory examination does not support diagnosis, CTscan examination in acute phase showed low sensitivity (approximately)16%; MRI is fairly senctive(83%) but is not universally available. Doctors in the ernergercy room need to improve their clinical skills.More than 50% patients with acute vestibular syndrome are associated with centra etiology; identification of vertigo symptoms anc neurologic focal signs is needed for more accurate management.
