Nefropati Urat Nata (Gouty Nephropaty)
ABSTRACT:
Gouty nephropathy or chronic uric acid nephropathy is a situation where uric acid or crystal is deposited on parenchyma and lumen of kidney tubule independently, causing chronic inflammatory response via direct injury, similar to formation of microtophus in other organs, causing interstitial fibrosis and finally kidney failure. Deposition of uric acid or crystal formed because of hypersaturation of uric acid or hyperuricaemia. Hyperuricaemia is caused by increased uric acid metabolism, decreased urinary uric acid excretion or both. Hyperuricaemia is associatedmith hypertension, vascular abnormality and kidney failure, direct injury mechanism theory is still controversial. Hyperuricaemia is an independent factor in IgA nephropathy, but not as a predictor of kidney function decline. If hyperuricaemia is independent factor to kidney failure, efforts to lower plasma uric acid will also lower kidney failure prevalence.
