Prealbumin as an Indicator of Nutritional Status in Children (Prealbumin sebagai Indikator Status Gizi pada Anak)
The purpose of this study is to get an illustration of serum prealbumin level in children and its relationship with anthropometrical nutritional status according to Gomez, Indonesia Tarwotjo-Soekirman and Depkes classifications. This study was done on 51 boys and 49 girls who were clinically healthy, aged 12 to 24 months, who visited the Growth and Development Clinics Health, Medical Faculty of Padjadjaran University/Hasan Sadikin General Hospital in the period of October-December 1993 with a certain criterion. Anthropometrical nutritional status according to the three classifications above were performed to all of the children. Blood samples were taken from each of the children to measure the level of serum prealbumin using a standard method. The results of this study revealed that mean serum prealbumin level was 14.94 mg/dl. No significant difference was found among boys and girls, i.e. 15.25 and 14.61 mg/dl, respectively (p = 0.239). Mild protein depletion (serum prealbumin level 10-15 mg/dl) according to Grant (1981) was found in 39 PEM children (70.9 percent) of Gomez classification, 11 children (20 percent) of Depkes classification and 1 child (1.8 percent) of Indonesia Tarwotjo-Soekirman (p=0.003). If mild PEM according to Depkes classification and Indonesia Tarwotjo Soekirman is also considered as PEM according to Gomez, the amount of children with mild protein became 34 (61.8 percent) and 14 (25.1 percent), respectively (p=0.012). Using serum prealbumin cut off point level of more than or equal to 16 mg/dl a significant difference among good nutritional status and mild PEM was found in Gomez classification (p=0.003) and Depkes (p=0.031) but not in Indonesia Tarwotjo-Soekirman (p=0.758). Based on serum prealbumin level, anthropometrical nutritional classification which represents the real condition of nutritional status were Gomez, Depkes. and Indonesia Tarwotjo-Soekirman classifications, respectively.
