Peranan Pharmaceutical Care Dalam Mengatasi Masalah Terapi Obat Pasien Usia Lanjut The Impact of Pharmaceutical Care Program in Old Patient Drug Theraphy

ABSTRACT
Pharmaceutical care (PC) is patient oriented, in the practice pharmacist collaborated with personal health care to improve clinical result and quality of life patient. Old patient is in the high risk condition to suffer drug related problem (DRP) because organ Mee, multiple disease and drug prescription. The fore evaluating PC to identify, to prevent and to soive DRP for old patient is impotant. Literature showed that program PC is very important to identit, to prevent and to solve DRP for old patient. Good Program PC conducted by well-trained pharma ist can identify, prevent, solve DRP and save health cost. Many drug prescription for old patient can improve potentialy of DRP Pharmacist collaboration with another health care in PC program to optimize therapy is important. PC Model that pharmacist made in hospital and long-term health care well accepted and give good benefit for patient. The conclusion from this article is PC program for old patient which conducted by a good pharmacist gave benefit not only in clinical but also in economic result. Old patient use cardiovascular drugs than another drugs. Pharmacist intervention models in the hospital or community for PC program gave benefit for patient and doctor.
