Penggunaan Trans Cranial Doppler untuk Deteksi Perubahan Hemodinamik Serebral pada Pasien Kritis
Cerebral haemodyamic changes is very common in patients with acute neurological insut. Neither angiographic or neuro-imaging studies can be performed in ICU setting. Case series from Bethesda TCD registry. The demographic and clinical data were obtained for all patients. The criteria of TCD finding was based on the standarized measures. TCD examination was performed using TD-DOP 9000 equipment with a 2-MHz probe for the examination of intracranial circulation. There were 234 TCD examination during the study period. Eighteen (7, 6%) of the TCD studies were performed in ICU setting. The cinical diagnosis is ischemic stroke (5 patients), post craniotomy for trauma (3 patients), hemorrhagic stroke (5 patients), and metoblic/ anoxic/ toxic encephalopathy (5 patients). The TCD findings are stenosis (6 patients), hipoperfusion (3 patients), increase ICP (3 patients), vasospasm (2 patients), and mixed (4 patients). Mobile TCD examinations can give valuable information about cerebral haemodynamic in critically patients. The examination can be performed in ICU setting safely for unconcious, bedriden, and ventilated patients.
