Transcrainial Doppler untuk Deteksi Perubahan Hemodinamik Serebral pada Stroke Akut.
Cerebral hemodyamic changes are very common in patients with acuta neurological insult. Neither angiograpic not neuroimaging studes can be performed in ICU setting. This is a cross sectional study. TCD examination was performed using TD-DOP 9000 equipment with a 2-MHZ probe for the examination of intranial circulation. MCA stenosis was diagnosed if peak systolic celocity (PSV)>140 diagnosed if peak systolic velocity (PSV)>140 cm/or mean systolic velocity (MSV).80cm/s. The stenosis of posterior circulation was defined when PSV>90 cm/s or MSV>60 cm/s. The data were taken from 234 patients, complete study was done in 182 patients. Ischemic stroke is the most common indication. Hypoperfusion was present in 21% cases. Stenosis present in 38% cases, more common in nterior ciculation. The most common risk factors are hypertension, dyslipidemia, and diabetes. Intracranial stenosis were more frenquently detected in patients with history of diabetes mellitus and hypertension. Moble TCD examination cangive valuable information on cerebral hemodynamic in acute be performed safely for unconscious bedridden, and ventilated patients.
