Correlation Between Asthma Control Test (ACT) and Spirometry as Toool of Asseesing of Controlled Asthma

Introduction: Asthma is a chronic inftammation disease, controlled asthma is a goal in management of asthma patients which essential as a parameter of successful therapy. To assessing controlled asthma the asthma control of patient accurately in a busy asthma clinic with limited time and resources is always a challenging to the clinician. Asthma Control Test (ACT) as a tool to assess of controlled asthma based on patients subjective answer or knowledge, need to be compared with spirometry which is an objectively tool. ACT consists of 5 queistionnaires which are patients do the assessment of their asthma status, a total score of 25 indicates complete controlled, total score less than 20 indicates of uncontrolled asthma. On the other side spirometry objectively measured airflow limitation in asthma patient. The aim of the study is to determine the correlation between the ACT and spirometry in assessing asthma control.
Methods: Study design is cross sectional and samples obtained by consecutive sampling in asthma clinic Persahabatan Hospital Jakarta. Patiens were requested to fill in ACT queistionnaire under supervised and followed a spirometric test. The result were analysed using descriptive statistics and Spe.trman correlation. Results: Amount of Samples are 100 patients consist of 70 womens and 30 mans by age between 18-50 years, mean of age 36.6 years. Subject consist of intermittent asthma are 22 patients, mild persistence asthma are 45 patients and moderate persistence asthma are 33 patients. There are 68 % of controlled and 32 % of uncontrolled. On analysis correlation of asthma degree with ACT score, obtained intermittent asthma are 95 % controlled, mild persistence asthma are 82,2 % controlled and moderate persistence asthma are 30,3 % controlled. The cut of point value of ACT in analysis Receivers Operating Characteristic (ROC) in differentiating FEV1 < 80 % to normal is ACT point 21.5 with 63.6 % sensitivity and 70.1 % specificity. The correlation of ACT and spirometri is denoted by spearman correlation coefficient is 0.382 with p value of 0.001. Conclusions: There is a statistically significant but weak correlation between ACT with spirometry, however ACT is useful to complete the role of spirometry in assessing asthma control.
