The Comparison of Examination Between Saline Inhalation Sputum and Alcohol Fixation Bronchial Washing with Seccomano Fixation for Lung Cancer Diagnosis

Background: Histopathological examination is paramount in patients with suspected lung cancer, because it is a gold standard for lung cancer diagnosis. Sputum cytology examination is the only non-invasive examination that can detect lung cancer, besides it is quite cheap and can be used widely.
Objective: The aim of this study is to compare whether any sensitivity differences among once 3% saline inhalation with alcohol fixation, continously within 3 days 3% saline inhalation with alcohol fixation and bronchial washing with alcohol fixation for lung cancer diagnosis.
Setting: In the ward unit of Dr. Moewardi general hospital Surakarta.
Methods: A total of 57 consecutive quota samples were examined once saline 3% saline inhalation with alcohol fixation, continously within 3 days 3% saline inhalation with alcohol fixation and bronchial washing with alcohol fixation for lung cancer diagnosis. The three ways were calculated for sensitivity and compared the value of the agreement and significancy. To compare the sensitivity of the screening method was used agreement test by calculating the kappa (k) and significant test by calculating the value of z.
Result: The continously within 3 days 3% saline inhalation with Saccomano fixation was more sensitive than once 3% saline inhalation with alcohol fixation. It has good agreement (k = 0.472) and significant (z calculated> z1 - .05). The continously within 3 days 3% saline inhalation with Saccomano fixation was less sensitive than bronchial washing with alcohol fixation. It has good agreement (k = 0.668) and significant (z calculated> z1 - .05). Once 3% saline inhalation with alcohol fixation was less sensitive than bronchial washing with alcohol fixation. It has poor agreement (k = 0.202) and significant (z calculated> z1-.05).
Conclusion: Sputum cytologic examination was done by doing continously 3 days 3% saline inhalation with Saccomano fixation can be recommended to be used for early detection of lung cancer screening.
