Penggunaan Skor 'Latch' sebagai Prediktor Keberhasilan Laktasi Pasca Persalinan Normal dan Bedah Sesar
A practical & concrete tool to predict breastfeeding outcomes following different modes of delivery is needed. The LATCH score provides a reliable, valid, and systematic method for gathering information about individual breastfeeding sessions.This study aimed to explore the LATCH scoring system in predicting breastfeeding patterns following caesarean and vaginal delivery.A convenience sample of healthy nursing mothers who had caesarean section and vaginal delivery during August 2009 - July 2010 were recruited from two pri¬vate hospitals in West Jakarta. Participants were first-time breastfeeding women, 18 years or older, with healthy singletons. LATCH scores were assessed on 24 hour post delivery, followed on 7 days and 6 weeks subsequently. Statistical analysis was carried out using SPSS version 16.0 for Kruskal-Wallis, Mantel Haenzel chi-square test, Spearman correla¬tion coefficient, and Relative Risk analysis. A sample of 117 mother-baby dyads who had a normal vaginal delivery, 100 with elective caesarean section and 162 with emergency caesarean section were recruited into this study. Total LATCH scores positively correlated with duration of exclusive breast-feeding (n=126, r=0.56, p=0.001) and mode of delivery (n=378, r=0.28, p=O.004). Low LATCH score at day-7 was the highest risk discriminate of the 3 time periods examined (p=0.025). Total LATCH score 0-2 encounters by 7 days of age was associated with breastfeeding less than 6 weeks for vaginal delivery (RR=3.4, 95%CI1.4-4.1), elective caesarean section (RR=6.2, 95%CI2.5-7.6), and emergency cae¬sarean section (RR=5.4, 95%CI 2.2-6.5). Conclussion of this study is The LATCH as¬sessment tool identifies as a predictor for the duration of exclusive breastfeeding followed caesarean and vaginal delivery.
