Removing normal appendix is an economic burden both on patient and health insurance, on the other hand misdiagnosis and delay in surgery may increase morbidity, mortality and health care cost. Alvarado score is one of the many scoring sistem for the diagnosis of acute appendicitis. Alvarado score is purely based on history (migration of pain, anorexia, nausea/vomiting), physical examination (tendermess in the right lower quadrant/iliac fossa, rebound pain, elevation of temperature), and few laboratory investigations (leucocytosis, shift to the left), and is very easy to apply. Many research of acute appendicitis was done with Alvarado score. Because of differential counting is not routinely done, some researchers compared Alvarado score and modified alvarado score. In their research. If Alvarado score or modified alvarado score ≥ 7, the patient is probable acute appendicitis and must rafer to hospital for surgery.
