The Histopathological Profile as Prognosis Factors on Squamous Cell Carcinoma of the Cervix which Underwent Radical Surgery (Profil Histopatologis sebagai Faktor Prognosis pada Karsinoma Sel Skuamosa Serviks yang Dilakukan Operasi Radikal)
Objective : To investigate the influence of histopathological profile such as histologic cell type, cell differentiation, vascular space invasion, lymphocyte infiltration, local metastatic term and necrosis, also pelvic lymph node metastases against the recurrent of on squamous cell carcinoma of the cervix stage IB-IIA which underwent radical surgery. Setting : Dr. Cipto Mangunkusumo Hospital, Jakarta, Indonesia. Material and methods : Investigated material were histoppathological speciment and medical record of patient which stage IB-IIA squamous cell carcinoma of the cervix which performed radical operation in January 1st 1986 until June 30th 1991. Ninety patients had been re-examined for histopathological speciments. Recurrent had been recorded to medical record if after 6 months operation or the patient loss of follow-up (historical cohort). Statistic analize by using stepwise analysis method. Results : Univariate analysis showed that cell differentiation, vascular space invasion, lymphocyte infiltration and pelvic lymph node metastasis significantly influence recurrence occur (p less than 0.05). Among those variables by using stepwise analysis method only cell differentiation and vascular space invasion significantly are prognostic factors. On the case positive vascular space invasion, the risk to occur recurrence 17.7 larger if compare without vascular space invasion and 2.5 larger if there was poor cell differentiation compared with well or moderate cell differentiation. Conclusions : Applying this significant risk factors, a novel strategy for managing patients after radical operation can be made better. Adjuvant therapy should be considered in high risk patients whether for radiation or chemotherapy or whether for both combination.
