Troponin-T Level on Unstable Angina Pectoris : Its Relationship to the History of Disease (Kadar Troponin-T pada Angina Pektoris Tak Stabil: Hubungannya dengan Perjalanan Penyakit)
Histopathologic findings showed microinfarctions or minor myocardial cell damage at the whole myocardium in patients with unstable Angina who developed fatal coronary events. Early detection of microinfarct which can not be detected by electrocardiogram or conventional cardiac enzymes is important for clinicians to do more aggressive intervention at the right time to lower morbidity and mortality due to unstable Angina Observational study has been performed within 60 days to find out Troponin-T level in Unstable Angina and its corelation with its natural history and to analyze the cut off point of troponin-T level for coronary events. We found that the range of Troponin-T level in Unstable Angina 0.00-8.39 ug/l, while in Stable Angina 0.00-0.20 ug/l. 59.46 percent of 37 Unstable Angina and 0 percent of Stable Angine developed coronary events. Among those who developed coronary events, 43.24 percent had Troponin-T more than 0.20 ug/l. There was 94.11 percent of Unstable Angina whose Troponin-T level more than 0.20 ug/l and 30 percent of those with Troponin-T level less than 0.20 ug/l developed coronary with Multiple Logistic Regression Analysis we found out that troponin-T level more than 0.20 ug/l was significant factor for detecting microinfarction/coronary events. The Odd Ratio for coronary events in Unstable Angina with Troponin-T level more than 0.20 ug/l is 9.3889 (95 percent C17.67-11.10). Conclusion: within a wide range Troponin T level in Unstable Angina, the significant level for early of microinfarction leading to coronary events is more than 0.20 ug/l.
