One of the important facilities in the community health care services for the high risk babies is improvement of the communication system. Home visit and care have to be done and it’s the responsibility of the Community Health Nurse’s and families. Home care it can be effective if there is a good communication system between Hospital, Community Health Centre and the families. This study is a descriptive research design which has a purpose to evaluate the communication system in Indonesian’s Health Care institution and the families. The sample consists of three group, namely mothers with high risk babies, nurses in the hospital and community health services in East Jakarta. The result shown in this study is that most of the mother’s of high risk babies perceived important home visit (83.16%), the mother’s perceived most of the nurses lack home care services (93,44%0
