Perbandingan Pemeriksaan Interferon Gamma Darah dengan Uji Tuberkulin untuk Diagnosis Tuberkulosis Paru di RSUP Persahabatan Jakarta
Until now, most of diagnosis requires detection of bacteria that ianosis TB. The TST was, until recently, the only test avilable for dcaused TB. If the bacteria are not found, we ned different tool to diagnosis of TB infection, but this test has limitation. Whole blod interferon gamma assay with ELISA have led to promising alernative.. Diagnostic test was perform to compare the accuration of blood IFN—y assay with tuberculin skin test in lung TB. Tuberculin skin test was performed using 2-TU dose of purified protein derivative RT 23 and the IFN-y assay was performed by measuring IFN-y response to EAST-6 and CFP-10. There wer 94 participants (47 confimed TB patients and 47 no identfiable risk of M.tb infection) completed the study. For the IFN-y assay with 0.35 IU/ml cutoff, the positive response rate in TB patients were 39 of 47 subject and in healthy volunteers were 21 of 47 sbject. For the TST with a 13 mm nduration cutoff, the positive response rate in TB patients were 38 of 47 subject and in healthy volunteers were 0 of 47 subject. Sensitivity, specificity, PPV the IFN-y assay were 83,0%, 55,3%, 65,0% dan 76,5%. Sensitivity, specificity, PPV and NPV the TST were 8,9%, 100% dan 83,9%. There were significants corrrelation (p=0,000, Spearman test) between blood test and skin test with weak correlation (r=0,286) and positive direction. Sensitivity IFN-y was better than TT, but specificity TST was better than IFN-y.
