The Use of Nitrit and Lekocyte Asterase for Screening Asymptomatic Bacteriura in Pregnant Woman (Penggunaan Tes Nitrit dan Tes Esterase Leukosit untuk Penapisan Bakteriuri Tanpa Gejala pada Wanita Hamil)

Objective : To evaluate the benefit of the use of nitrite and leukocyte asterase test for screening asymptomatic bacteriura in pregnat women at Dr. Cipto Mangunkusumo National General Hospital Jakarta. Design/Data Identification: Laboratory study diagnostic test. Data were obtained from February to April 1995. Sensitivity, specitivity, predictive value and accuracy of each test were evaluated. Setting : Department of Obstetrics and Gynecology and Laboratory of Department of Clinical Pathology, Faculty of Medicine University of Indonesia/ Dr. Cipto Mangunkusumo National General Hospital, Jakarta. Subjects, Patients, Participants : Three hundred pregnant women all of whom fulfill the criteria of acceptance and rejection. Intervention : Main outcome: Nitrite test and leukocyte esterase test results with urine as gold standard. Results: Asymtomatic bacteria were found in 22 subjects (incidence rate: 7.3 percent). Mean age is 26 years old with the most common age group between 26-30 years (36.7 percent). Most patients were primigravida (43.3 percent) and have been in the third quarter of pregnancy (63.3 percent). The type of patogene bacteria most commonly found in the urine culture with significant bacteriuria were: Acinetobacter calcoaceticus, Etenococcus and Escherichia coli (4 cases respective value and accuracy of nitrite test were: 13.6 percent, 99.6 percent, 75 percent, 93.7 percent, 85.7 perceent; the combination of nitrite and leukocyte esterase tests were: 86.3 percent, 85.2 percent, 32.7 percent, 98.7 percent, 85.3 percent respectively. Conclusions: Leukocyte esterase test can be used for screening asymtomatic bacteriuria in pregnant women because its sensitivity was reasonably good (86.3 percent), while nitrite test cannot be used because its sentsitivity in this study was exteremely low (13.6 percent).
