Central Nervous System Disturbances in Patient with Acute Gastroenteritis (Gangguan Neurologi pada Penderita Gastroenteritis Akuta)
The purpose of this study is to know the cause of CNS disturbances in patients with acute gastroenteritis. The study was carried out in the Pediatric Ward of Dr. Soetomo Hospital from January until December 1995. The patients population consist of 18 patients age between 1 month - 4 years. The level of dehidration, nutrition status, Blood Examination (WBC), cerebrospinal fluids and CNS disturbances were recorded, 8 patients (44.4 percent) have a good nutritional status, Blood Examination (WBC), cerebrospinal fluids and CNS disturbances were recorded, 8 patients (44.4 percent) have a good nutritional status, 2 patients (11,1 percent) with hypernatremia, 9 patients (50 percent) suffered from severe dehydration, 7 patients (38.8 percent) have acid base disturbances, 2 (11.1 percent) patients clinically showed sign of hypocalcemia. Shigella infection were observed in 3 (16.15 percent) patients, 4 patients (22 percent) died. Conclusion: Hupernatremia should be considered if we face a well nourished baby who suffered from acute gastroenteritis with high fever. If there are clinical sign of carpopedal spasm and shovsteks sign the diagnosis of hypocalemia should be considered although calcium serum is normal. Severe dehydration with CNS disturbances in obese patients have a bad prognosis. Matabolic encephalopathy have a better prognosis than encephalopathy due to abnormalites in CNS structure.
