The Management of Dengue Shock Syndrome (Penanganan Renjatan Demam Berdarah Dengue)
Dengue shock syndrome is a medical emergency that requires prompt and vigorous volume replacement therapy. Until now the mortality rate of dengue shock syndrome remains high especially when associated with encephalopathy. Data from Pirngadi Hospital Medan in 1994 showed a case fatality rate of dengue shock syndrome with encephalopathy of 60 percent. The case fatality rate of dengue shock syndrome in the Department of Child Health Dr. Soetomo Hospital Surabaya as well as Dr. Cipto Mangunkusumo Hospital Jakarta ranged from 2 to 26 percent. Clinical and pathological studies have shown that the major as manifestated by increase in the permeability of the general vasculature leading to hypovolaemia and haemorrhage. Trombocytopenia and abnormal coagulation are involved in the haemorrhagic diathesis. All of these accumulated facts lead to the need for proper management and better care. Based on the information mentioned above, a review of pathofisiology, pathogenesis and management of dengue haemorrhagic fever would be of benefit. An algorhythm of DHF grade III and IV would offer a good prospect to reach the ideal goal of managing severe cases of Dengue Shock Syndrome.
