Strategy to Eliminate Iodine Deficiency Disorders (IDD) by Using Innovative Primary Health Care Approach
It is estimated that today in Indonesia there are about 30 million people living in areas of iodine deficiency. There are approximately 10 million cases to goiter. 750,000-900,000 of cetinism and 3.5 million cases of iodine deficiency disorder (IDD). The prevalence of IDD, especially goiter has successfully been reduced by some methods of intervention. Thus. at the national level Total Goiter Rate (TGR) has decreassed from 37.2 percent (1982) - 27.7 percent (1990). However, in some areas the TGR is increasing. In is predicted that the elimination of IDD could increase national product by up to 15 percent or $ 100 (the curient GNP is $ 650). Or it can be stated that yearly loss caused by IDD in Indonesia. an innovative approach is being used. This innovative approach uses interrated primary health care. In the sub rural areas the provider of the health care. In the sub is posyandu (intergrated sub rural health care). Production and distribution (supply) of iodized salt is through the KUD (cooperative rural unit). The responsibility of coordination and supervision is carried out by the Health Center. NGOS like KIMIA FARMA (Industrial Farmacy) are also actively participating. In collaboration with SEAMED in Jakarta a longitudinal study has been begun. This uses an interdiciplinary approach, utilizing the disciplines of medicine/health, economics and agro complex one. The problem of Iodine Deficiency Disorder (IDD) in Indonesia is considered serious in which its direct impact on the quality of life, and human resources development is very great. It is estimated today more or less 42 million peoples are living in areas of iodine deficiency. The amount is getting increased compared to the estimation based on 1982's goiter survey, due to the population growth and the identification of new areas of IDD. From this amount there are araound 10 million cases of goiter. 750-900 thousands endemic creatine and 3.5 million cases of the other IDD. The population above might be found in 190 regencies of 26 provinces. Folowings are the total prevalence of IDD in each province (Total Goiter Rate) based on survey in 1982 and 1990.
