Incidence and Clinical Manifestation of Dengue on Adult in IPD Department, Dr. Saiful Anwar Hospital Malang, during January 1994 until December 1995 (Insidensi dan Manifestasi Klinik Penderita Demam Berdarah Dengue Dewasa yang Dirawat di Bagian IPD RSUD Dr. Saiful Anwar Malang selama Kurun Waktu Januari 1994 s/d Desember 1995)
Dengue Hemorrhagic Fever (DHF) which is caused by the dengue virus found to be endemic in Indonesia. Its morbidity and mortality rate were related to prevention measures, early diagnosis and prompt treatment. Prevention was effected by destroying the vector in the areas surrounding the patients and abiviously appropriate treatment could only be administered promptly after accurate diagnosis. Consequently, early diagnosis through clinical and laboratory examination as well as determination of the patients locations in order to effectively eradicate the vector were critical factors in reducing the morbidity and mortality rate of diseases. So far, other investigation in Indonesia almost always found the same clinical and laboratory manifestation but with various severity of the illness of dengue hemorrhagic fever at the Division of Internal Medicine of the Dr. Saiful Anwar Regional General Hospital, Malang, East Java, Indonesia. Fifty (9percent) of the patients admitted during this 2-years period of time between January 1994 - December 1995 (27 males and 23 females or 1.7 : 1) had the symptoms of DHF. Record of these fifty patients were analyzed. Ninety percent of the patients were between the ages of 12-29 years. Upon clinical examination, the patients were observed to have the following symptoms : fever and RL9+ in 100 percent spontaneous bleeding in 80 percent and hepatomegalic in 8 percent of the patients with less than 4 days of fever showed the following results : thrombocytopenic in 68 percent leucopenic in 48 percent and increase in PCV in 46 percent of the patients. Respectively, in terms of location 64 percent of the patients came from urban areas and 36 percent from rural areas. While in terms of time of the year, the most frequent occurence (44 percent) of DHF was found in the month between October and December. And finally, in terms of severity 85 percent of the patients were at the second grade and 16 percent were in the first. No death was reported. These findings were similar to those other investigations. However it is interesting to note that there were no third or more grade in our patients.
